Liability Waiver UF Club:
I am voluntarily participating in a class or classes provided by Ultimate Serenity LLC. I will be receiving instruction and information concerning fitness and wellness techniques, which may include weight training, Pilates, Couples Massage and other physical activities. I represent and warrant that I have no physical or mental health condition that would prevent my safe participation in these classes. I agree that if I am pregnant, or have a known cardiac arrhythmia (including very slow heart rate), a history of heart block, or if I am taking antipsychotic medications that may result in an adverse reaction in connection with physical activities, I will consult with and obtain the permission of a physician prior to engaging in any weight training or other physical activities in connection with these classes.
I am willingly and voluntarily assuming any risks, injuries or damages, known and unknown, which I might incur as a result of participating in these classes, and agree that Ultimate Serenity LLC will not have any liability for such injuries or damages, to the maximum extent allowed by applicable law.
The teacher, Lisa Wolkerstorfer, can accept no liability for personal injury or damages related to participation in a session online via zoom, in person or prerecorded on the members.ultimateserenitymassage.com website or the YouTube Channel. Exercise should be performed at a pace which feels comfortable for you.
I acknowledge and agree that the Ultimate Serenity staff are not medical professionals and have received training in physical activity programming for healthy individuals who do not require exercise in a medically supervised environment. If I experience dizziness, fainting, nausea, muscle cramping, or any other symptoms while exercising, I will discontinue the activity, notify Ultimate Serenity, and consult my physician.
On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Ultimate Serenity, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Ultimate Serenity program.
I expressly waive all rights afforded by any statute which limits the effect of a release with respect to unknown claims.
